WAIVER RELEASE FORM

Tournament & Academy Conditions

  1. As the Meadow Brook Polo Club & Academy is officially an active USPA Member Club, all Club and Academy players, students and participants must be current with their 2026 USPA Membership Dues and must follow the Association’s NOCSAE Helmet requirements in order to participate in ANY equestrian related activities at the Meadow Brook Polo Club & Academy.

  2. Per the USPA’s Membership requirements, ‘All members, except Non-Competing Members are required to be affiliated with a USPA Club in order to be a USPA Member.’

    We kindly ask all new and existing students, members, players and participants to support the Meadow Brook Polo Club & Academy by selecting the MB POLO CLUB as your USPA Club Affiliation. 

  3. All players competing in any USPA tournaments at the Meadow Brook Polo Club & Academy must have a USPA registered handicap.

  4. We recommend reviewing the 2026 Official Rulebook of the United States Polo Association  for more information on rules, policies and tournament conditions.

  5. All players and students at the Meadow Brook Polo Club & Academy MUST sign the Waiver Release Form in order to participate in any equestrian and/or non-equestrian related activities.
